Submitting a report is easy….click on the link, fill out the top portion, add any media you have, and it’s done! Or if you prefer to submit on your phone, scan the QR code.

To make a report, click on the link above to be taken to the reporting form. All reports are important, so fill it out to the best of your knowledge. Please be sure to supply your name and contact information so we may contact you if we have any questions. The most important thing you can provide are any media (photos or videos) of what you saw so please submit them if you have any. Note that any fields with an asterisk * is required information.

Thank you for participating in and contributing to Eyes of the Reef. Reports are used to alert the Hawai‘i Division of Aquatic Resources of potential threats to coral reefs around the state and provide information that informs potential intervention actions if necessary.
All reports are available to the public in the map dashboard embedded and linked below. This product provides location information, photos, and survey responses tied to each report so that users can look for reports they or others have submitted. This dashboard allows users to filter reports based on island, reporter category, and report topic. Additionally, clicking on individual entries on the left of the dashboard will take the user to that location on the map as well as show the report survey responses. All identifying information will not be viewable through the dashboard for anonymity. If you would like more information on individual reports, please contact us.
